We try to answer your questions in advance to avoid confusion. Respectful communication with you will be critical to your quality vision care from our office.
Insurance Billing Information: As a courtesy, Mt. Hood Vision Center will verify and bill your insurance; both primary and secondary if applicable. When making your appointment, please be sure to have your vision and medical insurance information available including the name of insurance company(s), ID number, and contact phone number. This will enable our staff to verify your coverage. If we find out there are no insurance benefits or you are not eligible for your benefits, we WILL call you.
We take pride in helping you through the complexities of understanding what to expect from your insurance and helping you to make your insurance billing easier. In most cases we bill your insurance for you.
Please keep in mind that not all insurance companies include vision on all of their plans. In many situations we are able to bill your care to your medical insurance if there is a medical complaint or concern or problem.
The following is a list of insurance plans in which we participate. If you do not see your insurance plan listed, call our office and we will verify if we accept it. We may be able to bill as an out of panel provider if you choose to our services when we are not on your pannel.
Our list of providers often changes so don't hesitate to give us a call.
Full payment of co-pays, deductibles, services and materials not covered by insurance will be required at the time of service. If you are unable to pay in full at the time of service, payment arrangements can be made in the form of a payment plan. Please talk to our staff to make arrangements in advance.
We also participate in the Care Credit program. If you are not familiar with this program, it is a program that provides financing with many options for payments, such as 90 days no interest or 12 months no interest. If this program sounds intriguing to you, please call our office and we will gladly send you information.
Accounts 30 days old are considered delinquent, when insurance is not a factor. Although we do not charge finance charges or a handling/rebilling fee, we will, after 90 days, turn your account over to our collection agency.
Checks returned for insufficient funds, closed accounts or other problems, will be subject to a $25 charge.
